Tom Dahlborg, MHSM

• MI-CCSI Executive Director
• Forty years of extensive healthcare leadership experience with a background in quality improvement, analytics, research, finance, and more.
• Named a Healthcare Disruptor and a Mental Health Champion by Authority Magazine and a Ruckus Maker by The Patient Experience Community.
• Directs all activities of the organization within the framework of the Mission and Vision Goals and Values as set by the MI-CCSI board.

Phil Baty, MD

• MI-CCSI Medical Director
• Family Medicine physician for nearly 30 years with an advanced understanding of improving medical care for patients with multi-morbidity, particularly those with behavioral health conditions and chronic.
• Fosters patient medical home concepts to ensure the voice of primary care and patient is incorporated into all applicable organizational initiatives.
• Provides clinical oversight and advises on medical content to ensure organization adherence to mission and values and fidelity to evidence-based practices.

Sue Vos, RN, BSN

• MI-CCSI Program Director
• President of the Case Management Society of America: Grand Rapids/Kalamazoo Chapter.
• Extensive experience in working with multi-disciplinary teams to provide care for complex patients, implementing care models, driving quality improvement strategies, training, and more.
• Leads program development and implementation of all training and quality improvement initiatives designed to achieve betterment for health systems, care teams, and patients and families.

Amy Wales, BS

• MI-CCSI Operations Manager
• A recognized solution finder who brings stability to situations and organizations.
• Extensive experience implementing and monitoring provider value-based incentive programs.
• Leads operations including human resources, accounting, technology, data and analytics, marketing and communications.

Pauline Virro-Nic, MBA

• MI-CCSI Project Manager
• Fifteen years dedicated to supporting and improving learning through project management and innovation in healthcare.
• Known for flexibility and problem-solving, supports improvements and standardizations to processes and systems.
